If you are a new patient and need a repeat prescription you will need to see a doctor first. In future you will then need to return your repeat slip to the surgery, posting in one of our boxes. If the slip has been mislaid forms are available in the surgery for completion or a note with your full name and address and the name of the drugs you require is acceptable.

Please indicate at the bottom of the request if you require one of our authorised chemists to pick up your prescription.

Please note we do not accept requests for prescriptions over the phone.

You can now order your repeat medication online but you will need a username and password to use this service, please ask the receptionist.
